About
A decreased secretion of thyroid hormones leading to inadequate or decreased tissue and organ metabolism; can be a result of surgery to remove an overactive thyroid or to treat hyperthyroidism with radioactive iodine.
Assess
Weight gain, severe fatigue, decreased hair growth, low TH levels, high TSH levels, cold intolerance, edema all over, thick tongue.
Complications
Myxedema coma; dysrhythmias; death
Diagnosis
- Risk for infection related to impaired wound healing, circulatory changes
- Ineffective breathing pattern related to fatigue and obesity
- Imbalanced nutrition: more than body requirements related to decrease in metabolism
- Activity intolerance related to abnormal thyroid function
Goals
- The client will remain free from injury.
- The client will demonstrate understanding of medication therapy and treatment.
- The client will identify methods to conserve energy and balance rest with activity.
Interventions
- Monitor VS, cardiac rhythm, pulse oximetry.
- Monitor for lethargy, confusion, memory loss.
- Monitor blood sugars, thyroid hormone labs, electrolytes, temperature at the start of treatment.
- Administer medications ( synthroid, levothroid, etc.) as ordered.
- Teach client about the side effects, rationale, and dosing schedule for medications.
- Monitor for signs/ symptoms of complications.
