About
Viral infection of the skin causing either cold sores in and around the mouth ( HSV-1) or genital herpes ( HSV-2), characterized by periods of remission and exacerbation.
Assess
HSV- 2; isolated or grouped vesicles on skin or genitourinary tissues. HSV-1; mouth numbness or tingling, cold sore. Recurrent exacerbations and remissions.
Complications
Transmission to others; ineffective coping
Diagnosis
- Impaired skin integrity related to ulceration and lesions of skin surfaces
- Acute pain related to lesions
- Impaired urinary elimination related to pain with urination
- Ineffective health maintenance related to deficient knowledge of disease process
Goals
- The client will identify ways to prevent the spread of infection.
- The client will have resolution of lesions.
Interventions
- Monitor pain levels and administer prescribed analgesics as necessary.
- Provide sitz bath, perineal care as needed.
- Administer antivirals as ordered.
- Teach the client about medications including rationale, side effects, and adverse reactions.
- Encourage client to verbalize feelings regarding disease.
- Teach client about disease process, manifestations of exacerbation, and prevention of transmission.
- Teach client about risks of neonatal transmission.
- Encourage client, during acute outbreak, to avoid sexual intercourse.
- Monitor for signs/ symptoms of complications.
