About
Damage to the ocular nerve resulting from increased intraocular pressure. Inability of aqueous humor to flow out from the behind the eye due to impaired "plumbing" of the eye ( open-angle glaucoma) or due to orbital trauma blocking the flow of aqueous humor ( closed-angle glaucoma)
Assess
Vision loss or changes; tonometry to measure the intraocular pressure; opthalmoscopy to visualize the optic nerve and assess for damage; gonioscopy to measure the angle from the anterior chamber; perimetry to assess peripheral disturbances
Complications
Blindness, increased visual impairment
Diagnosis
- Acute pain related to headache and brow pain.
- Sensory deficit related to visual impairment.
- Knowledge deficit related to disease and management.
- Risk for injury related to decreased vision.
Goals
- The client will be able to identify surroundings and landmarks.
- The client will remain free from injury.
- The client will verbalize a decrease in pain less than 4 on a 0-10 pain scale.
- The client will identify medication rationale, side effects, and regimen.
- The client will demonstrate how to administer eye drops.
Interventions
- Assess pain levels on a scale of 0-10.
- Assess client visual acuity and level of visual impairment.
- Assist client with identifying surroundings if visually impaired.
- With meals, describe plate using a clock to delineate where specific foods are located.
- Administer beta blockers, miotics, carbonic anhydrase inhibitors as prescribed by physician ensuring proper eye drop administration. Wait five minutes in between each eye drop medication. Press on the lacrimal duct for 1-2 minutes after eye drop administration to prevent systemic absorption and side effects.
- Teach client how to administer eye drop medication.
- Observe client administering eye drop medication ensuring adherence to appropriate technique.
- Teach client side effects and adverse reactions of medication therapy.
- Encourage client to report any adverse effects of medications.
