About
A variety of injections given to encourage passive immunity to diseases and illnesses.
Assess
History of vaccination or immunizations; allergies to previous vaccination; knowledge regarding vaccinations; allergy to eggs may contraindicate vaccination; pregnancy may contraindicate some vaccinations; assess site of injection for localized rash; monitor for hypersensitivity, fever, rash.
Complications
- Allergies- monitor for shortness of breath or difficulty breathing, seek immediate medical intervention if these symptoms arise.
- Temperature- may give over the counter antipyretics ( acetaminophen, ibuprofen) after checking with health care provider.
- Infection- avoid live viruses in immunocompromised individuals or those in close contact with immunocompromised individuals.
Diagnosis
Deficient knowledge related to vaccination purpose, side effects, and potential complications
Goals
The client will state rationale for vaccination and signs and symptoms to report to health care provider.
Interventions
- Assess allergies of the clients, as some allergies may contradict vaccination.
- Assess the clients previous reaction to vaccines.
- Assess immune status and persons in close contact with immunocompromised individuals; this may contraindicate live attenuated vaccines.
- Record information about the vaccination ( Lot, batch #, manufacturer, dose, site given, etc) in the clients records and give the client copies of those records.
- Teach the client about each vaccination, including rationale for use, potential side effects, potential adverse reactions, and when to follow up with health care provider.
- Teach client about expected reactions, versus unexpected reactions.
- Monitor for signs/ symptoms of complications.
