Skin lesions, irregular borders, blue, white, or red abnormal coloring, nodular; need biopsy confirmation

Death, metastasis
Diagnosis
- Ineffective coping related to diagnosis.
- Knowledge deficit related to disease progression, treatment options, management.
- Acute pain related to skin graft/ biopsy procedures.
- Fear related to threat of death.
Goals
- The client will have a pain of less than 4 on a VAS 0-10 pain scale.
- The client will be able to identify disease course, management and treatment options.
- The client will discuss concerns and fears related to treatment and disease process.
Interventions
- Assess skin areas involved, knowledge about disease, pain, itching, redness, changes in moles or skin growths, tenderness.
- Administer analgesics as indicated by physician.
- Encourage client to report pain levels at or above level 3 on a scale of 0-10.
- Encourage client to discuss feelings associated with disease and treatment.
- Teach client about therapies, treatment, side-effects.
- Provide supportive care and encourage opportunities for client to discuss wishes for care if disease advances.
- Teach client to use high SPF sunscreens and cover body areas if out in the sun.
