236-Brain Abscess

About
A collection of pus that can collect within areas of the brain usually caused by indirect or direct invasion of bacteria into the brain.
Assess
Headache, fever, lethargy, confusion, pupillary changes, airway and respiratory changes, motor, cognitive and sensory changes with progression
Complications
Increased intracranial pressure; septic emboli; disseminated Intravascular coagulation ( DIC); death
Neurological deficits
Diagnosis
  • Risk for injury related to brain infection
  • Acute pain related to increased cerebral edema and inflammation
Goals
  • The client will be free from complications.
  • The client will verbalize understanding of treatment plan.
Interventions
  • Monitor vital signs, neurological status, and circulation to extremities.
  • Monitor for s/s of increased intracranial pressure ( seizures, non-responsiveness, nausea, vomiting, hypertension, bradycardia, changes in neuro status).
  • Initiate seizure precautions.
  • Keep in a quiet, darkened room to manage for photophobia.
  • Administer medications ( antibiotics, analgesics, etc.) as ordered.
  • Monitor for signs/ symptoms of complications.
