About
Transfusion of bone marrow from donor to cancer patient. Once the bone marrow settles, it can reproduce healthy rbc's, wbc's and platelets that will aid in attacking any abnormal cells still left within the body. Bone marrow transplant can also be infused after killing all remaining bone marrow in a recipient with radiation and/or chemotherapy, so that new, noncancerous cells can reproduce and protect the body while removing all cancerous cells.
Assess
Vitals frequently before, during, and after treatment; CBC, electrolytes, IV site, tolerance to transplant, appetite, pain, nausea, side effects, understanding of procedure, coping mechanisms, intake and output, signs of complications such as GVHD ( itching, rash, hepatomegaly, nausea, diarrhea, increased liver enzymes)
Complications
Graft-versus-host disease, infection, liver failure, death
Diagnosis
- Fatigue related to immune system compromise.
- Risk for infection related to immunodeficiency.
- Risk for injury related to impaired immune defenses.
- Fear related to threat of death.
- Nutrition less than body requirements related to impaired oral intake.
Goals
- The client will remain free from infection.
- The client will maintain adequate nutrition.
- The client will be able to discuss fears, concerns related to diagnosis.
- The client will maintain intact skin integrity.
- The client will be able to discuss anxiety related to body changes, hair loss.
- The client will have adequate pain relief.
- The client will have an increase in activity tolerance.
- The client will remain free from complications.
Interventions
- Monitor for signs and symptoms of infection.
- Administer anti-emetics, pain medicines as needed.
- Administer irradiated blood products to decrease risk of GVHD.
- Encourage small, frequent high caloric meals.
- Encourage client to express fears and concerns related to diagnosis.
- Encourage adequate fluid intake.
- Avoid sick personnel, visitors.
- Monitor IV sites frequently, assess for blood return prior to administering blood products.
- Report any increase in temperature, rash, pain, achiness, diarrhea as may indicate complication.
