About
Decreased chloride below 97 mEq/L.
Assess
Acid-base disturbances, electrolytes disturbance concurrently. Tetany, increased reflexes, weakness, twitching, cardiac dysrhythmias, coma, seizures, water excess.
Complications
Metabolic alkalosis, additional electrolyte disturbances, coma, death.
Diagnosis
- Risk for injury related to fluid/ electrolyte imbalance
- Knowledge deficit related to dietary necessities.
Goals
- The client will have electrolytes within acceptable ranges.
- The client will remain free from injury and complications.
- The client will state foods that have high content of chloride.
Interventions
- Monitor for orientation, changes in level of consciousness, changes in muscle strength.
- Monitor electrolytes, arterial blood gases, intake and output.
- Administer IV infusions to increase chloride levels as indicated by physician.
- Teach client foods high in chloride that may help with imbalance including bananas, eggs, cheeses, canned vegetables, etc.
