About
Excess phosphorus > 4.5mg/dL. Most common contributing factor is renal failure.
Assess
Tetany, low calcium levels, muscle weakness, joint and bone pain.
Complications
Calcification in joints, cartilage, and bones; decreased to absent urine output; cardiac palpitations, impaired vision. Tetany due to hypocalcemia.
Diagnosis
- Risk for injury related to fluid/ electrolyte imbalance
- Urinary elimination altered related to increased phosphorus levels
Goals
- The client will remain free from injury.
- The client will have at least 30 cc's/ hour of urine output.
- The client will have electrolytes within acceptable ranges.
Interventions
- Assess electrolytes, especially calcium and phosphorus.
- Monitor intake and output.
- Teach the client to monitor urine output and report if less than 30cc's/ hour.
- Monitor for tetany.
- Teach the client signs and symptoms of tetany that need immediate follow-up.
- Encourage client to report palpitation or any other signs and symptoms that may indicate a worsening of condition.
