Inflammatory reaction to the skin surface due to contact with a physical, biological, or chemical agent.
Assess
Redness, abrasions, burning, itching, edema, blistering, hives, pustules, vesicles, drainage, crusting, dryness, peeling
Complications
Infection, severe inflammation, scarring
Diagnosis
- acute pain related to inflammatory reaction.
- Altered comfort related to itching, irritation, burning.
- Impaired skin integrity related to open lesions.
- Risk for infection related to break in skin integrity.
Goals
- The client will state pain level acceptable after intervention.
- The client will identify strategies to promote comfort and soothing of skin surfaces.
- The client will remain free form infection.
- The client will have evidence of decreasing skin involvement and healing of skin surfaces.
Interventions
- Assess skin surface areas.
- Monitor for infection and teach patient signs and symptoms to report indicating infection.
- Use mild agents to clean skin areas ( no soap).
- Apply cool compresses to involved areas.
- Use room temperature baths with medication ( as prescribed) for large surface area involvement.
- Apply lotion or cream agents as prescribed in a thin layer.
- Apply topical ointments, including hydrocortisone creams, as prescribed and teach client how to apply using aseptic technique.
- Administer oral steroids as prescribed for severe inflammation.
- Teach client treatment plan and interventions.
