260-TIA (Trans Ischemic Attack)

About
Small, reversible periods of lack of blood supply to areas of the brain.
Assess
Blurred vision, weakness and numbness that comes and goes in arm or leg, slurred speech, difficulty talking, visual disturbances.
Complications
Cerebral vascular accident
Diagnosis
Ineffective health maintenance related to deficient knowledge of disease process and risks
Goals
  • The client will be free from injury.
  • The client will verbalize signs and symptoms requiring immediate follow-up.
Interventions
  • Teach client signs and symptoms of stoke requiring immediate follow-up.
  • Teach client about drug therapy and modifiable risk factors ( smoking cessation, blood pressure management, heart healthy diet, etc.) to implement to prevent reoccurrence or stroke.
  • Monitor onset, severity of symptoms.
  • Monitor vital signs, neurological checks.
  • Prepare for additional diagnostic tests as indicated.
  • Teach client when to seek emergent medical attention.
  • Monitor for signs/ symptoms of complications.
