Diagnosis
- Impaired skin integrity related to redness and lesions on body
- Disturbed body image related to changes in physical appearance
Goals
The client will verbalize understanding of disease and treatment modalities.
Interventions
- Monitor involved skin areas.
- Encourage client to verbalize feelings about body image changes and disease process.
- Administer medications ( Tar therapy, immunosuppressants, topical corticosteroids, etc.) as indicated.
- Teach client about medications, rationale for use, and potential side-effects.
- Provide emotional support for the client coping with the disease.
- Teach client to monitor for infections with immunosuppressive therapies.
- Monitor for signs/ symptoms of complications.
