Diagnosis
- Sensory deficit related to visual impairment.
- Knowledge deficit related to disease and management.
- Risk for injury related to decreased vision.
Goals
- The client will maintain good blood glucose control.
- The client will maintain an annual schedule of meeting with ophthalmologist.
- the client will identify techniques to aid with visual impairment.
- The client will remain free from injury.
- The client will identify strategies to prevent worsening of condition.
Interventions
- Assess and monitor visual acuity.
- Monitor blood glucose levels and maintain a thorough insulin schedule to accommodate for increased blood sugar levels.
- Monitor Hemoglobin A1C 3-4 times a year to assess level of sugar control.
- Teach client importance of maintaining good glycemic control.
- Teach client the necessity of maintaining annual ophthalmologist screenings.
- Encourage client to follow-up sooner if visual disturbances or other complications.
- If visual impairment, teach client how to locate landmarks, prevent rearrangement of furniture, and encourage client to use adaptive tools ( i.e. cane) to aid in movement and decreasing injury.
- Monitor for complications.
