Decrease in sodium, below normal levels.
Assess
Fluid volume status; Neuro, confusion, headaches; skeletal muscle weakness, tachycardia; nausea, diarrhea
Complications
Coma, seizures, death
Diagnosis
- Impaired memory related to fluid and electrolyte imbalances
- Impaired mobility related to muscle weakness
- Acute pain related to increased gastrointestinal motility, abdominal cramping, and diarrhea
Goals
- The client will remain free from injury.
- The client will have sodium levels increased above 135 meq/L ( may have different parameters for each clinical agency).
Interventions
- Monitor intake and output, VS, and neurological checks frequently.
- Assess muscle strength, including those of the respiratory system, to ensure adequate respirations.
- Monitor electrolytes and signs for fluid volume excess frequently.
- Institute fall precautions and injury prevention strategies.
- Administer IV fluids, with fluid deficit, as ordered.
- Monitor for signs/ symptoms of complications.
