257-Retinal Detachment

About
Separation of the retinal pigment epithelium from the sensory layer requiring surgery in most cases.
Assess
Vision changes including shading over the eye, blurred vision, appearance of floaters, flashing lights, etc.
Complications
Increased intraocular pressure, cataracts, additional incidence of retinal detachment, visual disturbances
Diagnosis
  • Sensory deficit related to visual impairment.
  • Risk for injury related to sensory deficit.
  • Knowledge deficit related to treatment and surgical interventions.
Goals
  • The client will maintain adequate vision to perform ADL's.
  • The client will describe surgical intervention, complications, and post-operative care.
  • The client will remain free from injury.
Interventions
  • Prepare the client for surgical intervention.
  • Teach the client what to expect pre-, intra-, and post-operatively.
  • Maintain the client in a prone position post-operatively to allow or healing and decreased bleeding.
  • Teach the client complications that require immediate follow-up.
  • If visual impairment, teach client how to locate landmarks, prevent rearrangement of furniture, and encourage client to use adaptive tools ( i.e. cane) to aid in movement and decreasing injury.
  • Monitor for complications.
