About
Increase in sodium, above normal levels.
Assess
Neurological status; Hypervolemia or hypovolemia; assess for pulmonary edema if excess fluid volume; agitation, confusion, coma, lethargy, monitor I/O.
Complications
Injury; coma; seizures
Diagnosis
- Impaired memory related to fluid and electrolyte changes
- Activity intolerance related to muscle spasms, twitching and weakness
Goals
- The client will have decreased sodium, below 145 meq/L ( may have different parameters for each clinical agency).
- The client will remain free from injury.
Interventions
- Institute fall precautions to prevent injury.
- Monitor cognitive function, vitals, fluid balance, intake and outputs.
- Teach the client dietary considerations, especially to avoid foods high in sodium content.
- Administer hypotonic IV solutions or diuretics as ordered by a physician.
- Monitor for signs/ symptoms of complications.
