173-Varicose Veins

About
Abnormally dilated superficial venous vessels, most often occurring in the lower extremities.
Assess
Raised veins on the skin surface areas, pain, dull aches, muscle fatigue in the lower extremities, edema.
Complications
Thromboembolism
Diagnosis
  • Ineffective tissue perfusion related to incompetent veins.
  • Pain related to dilated vessels.
  • Body image disturbance related to superficial leg discoloration and swelling.
  • Risk for injury related to incompetent veins.
Goals
  • The client will identify methods to decrease the risk of infection and injury.
  • The client will have adequate peripheral circulation as evidenced by positive pedal pulses.
  • The client will describe methods to remove varicosities and potential for complications related to procedures.
  • The client will have a pain of less than 4 on a VAS 0-10 pain scale.
  • the client will maintain skin integrity and have no signs of infection.
Interventions
  • Assess the extremities, especially for signs and symptoms of impaired tissue perfusion ( paralysis, parasthesias, pain, pulselessness, pallor).
  • monitor for breaks in skin integrity or for unilateral swelling or pain in extremity indicated possibility of blood clot.
  • Administer analgesics as prescribed for pain or discomfort.
  • Teach client methods to decrease risk of future varicosities ( antiembolism stockings, avoid prolonged standing, weight loss if obese, light activity, avoid crossing legs, etc).
  • Teach client about procedures to help eliminate or reduce the appearance of varicosities.
  • Teach the client signs and symptoms that require immediate follow-up ( skin changes, cyanosis of the extremities, thromboembolism signs/symptoms).
  • Encourage client to talk about feelings related to discoloration and appearance of extremities.
