About
Disease of the heart muscle causing changes that effect the ability of the heart muscle to efficiently pump blood.
Assess
Arrhythmias, dyspnea, fatigue, activity intolerance, edema, pulmonary congestion, syncope, angina, weakness
Complications
Heart transplant; heart failure; death
Diagnosis
- Impaired gas exchange related to inability for heart to meet metabolic demands
- Decreased cardiac output related to pump failure
- Activity intolerance related to exertional dyspnea and fatigue
Goals
- The client will remain free from injury.
- The client will state understanding of disease treatment and symptom management.
Interventions
- Administer oxygen as ordered.
- Monitor daily weights.
- Monitor vital signs, telemetry, fluid balance, intake and outputs, pulse oximetry.
- Administer medications ( diuretics, antidysrhythmics, cardiac glycosides, beta blockers, etc.) as ordered.
- Teach client disease treatment, symptoms to report to health care provider ( dizziness, palpitations, pain, dyspnea), and medication therapy.
- Monitor for signs/ symptoms of complications.
