About
Surgical procedure utilized to widen the arteries to the brain that were previously obstructed or narrowed.
Assess
Incision, bleeding, bruising, level of consciousness, Vital signs, pulse oximetry, carotid pulses, pain.
Complications
Stroke, intracranial bleeding, post-operative complications ( infection, pneumonia, paralytic ileus, DVT's, PE, etc)
Diagnosis
- Knowledge deficit related to perioperative treatments
- Acute pain related to invasive procedure and incision
Goals
- The client will verbalize understanding of pre-, intra-, and post-operative instructions and activities.
- The client will remain free from injury.
Interventions
- Teach the client what to expect pre-, intra-, and post-operatively.
- Monitor vital signs post-operatively every 15 minutes X 2, every 30 minutes X 2, every hour X 2, and every 4 hours ( may have different guidelines for each clinical agency).
- Monitor incision for bleeding bruising, swelling and notify physician immediately.
- Monitor neurological status and peripheral pulses frequently.
- Monitor extremity including edema, redness, pulses, pallor, pain, and decreased sensation.
- Administer medications as ordered.
- Teach client s/s to report to health care providers.
- Monitor for signs/ symptoms of complications.
