About
Inability to normally eliminate feces, difficulty passing stool, or hard, small stools.
Assess
Previous bowel elimination patterns, date of last bowel movement, bowel sounds, quantity and characteristics of stools, diet history.
Complications
- Bowel obstruction- lack of passing soft formed stools, decreased to absent bowel sounds
- Laxative abuse- encourage non-pharmacological management techniques.
Diagnosis
Constipation r/t immobility, diet, fluid intake, etc
Goals
Client will verbalize decreased abdomen discomfort and passing of soft formed stool.
Interventions
- Assess abdomen, monitor stool pattern.
- Encourage activity to increase peristalsis.
- Administer laxatives, cathartics, enemas as ordered.
- Educate client to increase foods high in fiber.
- Educate client to increase fluids at least eight 8oz glasses of water daily ( if no fluid restrictions).
- Monitor for signs/ symptoms of complications.
