About
Inflammatory disease with periods of remission and exacerbation affecting the small intestine and/or the colon.
Assess
Abdomen- bowel sounds, distention, tenderness, stool patterns, specimen for stool culture and fecal occult blood.
General- weight loss, fluid volume status, psychosocial
Complications
Perforation, gastrointestinal bleeding, dehydration, fistula formation, bowel obstruction, ineffective coping
Diagnosis
  • Risk for fluid volume deficit r/t diarrhea
  • Diarrhea r/t inflammatory process
  • Pain r/t inflammatory process
Goals
  • Client will state decrease pain or pain management effective.
  • Client will state a decrease in diarrhea and abdominal cramping.
Interventions
  • Maintain client NPO until resolution of diarrhea.
  • Monitor amount and consistency of stool and frequent vital signs.
  • Administer immunosuppressants and metronidazole as ordered.
  • Administer TPN and fluids/ electrolytes via IV as ordered.
  • Administer analgesics/ narcotics prn as ordered.
  • Encourage client to verbalize feelings regarding disease process, give information for support. services available.
  • Monitor for signs/ symptoms of complications.
