105-Colorectal Cancer

About
Adenocarcinoma, usually, in the colon and/ or rectum.
Assess
Abdominal discomfort, change in bowel habits, diarrhea, constipation, black tarry stools, blood in stools, anorexia, anemia, rectal pain.
Complications
Bowel obstruction, perforation, peritonitis, sepsis, metastasis, death
Diagnosis
  • Anticipatory grieving related to diagnosis and treatment.
  • Knowledge deficit related to diagnosis and treatment.
  • Fluid volume deficit related to intestinal losses.
  • Nutrition less than body requirements related to impaired oral intake.
Goals
  • The client will state a decrease in abdominal pain and discomfort.
  • The client will be able to identify treatment options and possible complications.
  • The client will maintain adequate fluid balance.
  • The client will be able to discuss feelings related to diagnosis and treatment.
  • The client will be able to maintain nutrition.
  • The client will be free from complications of surgery.
Interventions
  • Assess bowel patterns, intake and output, electrolytes, pain levels, vital signs.
  • Monitor fluid and electrolytes.
  • Prepare client for surgery, as indicated.
  • Encourage client to discuss feelings related to diagnosis and treatment.
  • Teach client expectations of procedures and treatments.
  • Teach client ways to decrease risks of complications ( Coughing and deep breathing, adequate fluid intake, adequate caloric intake, ambulation, signs and symptoms that require follow-up)
  • See colon resection and colostomy if procedures are performed for the client with colorectal cancer.
