About
Anemia caused by inadequate consumption of vitamin B12, which impairs transport of folic acid.
Assess
Fatigue, pallor, weight loss, glossitis, tingling or numbness in the hands or feet, impaired gait and/ or balance, Dietary history ( vegetarian and lactose free diets at high risk).
Complications
Parasthesias; impaired gait
Diagnosis
Imbalanced nutrition less than body requirements related to dietary deficiency of vitamin B12
Goals
- The client will be able to state food s high in vitamin B12 content.
- The client will remain free from injury.
- The client will be able to identify medication regimen with side effects and rationale for each medication.
Interventions
- Monitor for fatigue, parasthesias ( numbness and tingling in extremities) and impaired gait.
- Encourage client to eat foods high in Vitamin B12 and folic acid.
- Administer supplements as ordered, administer IM injections if pernicious anemia is present.
- Teach client dietary sources high in Vitamin B12 and folic acid.
- If client is vegetarian, assist client in finding alternate sources to meet vitamin B12 needs.
- Monitor for signs/ symptoms of complications.
