206-Thrombocytopenia

About
A decrease in platelets, causing impaired coagulation.
Assess
Low platelets, bruising, petechiae, any current injuries, assess for signs of active bleeding, hypotension.

Hemorrhage

Injury
Diagnosis
Risk for injury related to increased risk of bleeding and decreased platelet count
Goals
  • The client will remain free from injury.
  • The client will be able to identify medication regimen with side effects and rationale for each medication.
Interventions
  • Monitor vital signs and lab results-- notify physician if hypotensive or changes in CBC indicative of blood loss.
  • Monitor for acute bleeding, including bruising, black stools, bleeding gums.
  • Encourage client to call for help when ambulating if feeling weak, dizzy, or fatigued.
  • Teach client about medications and/ or treatments, including platelet transfusions, according to regimen.
  • Monitor for signs/ symptoms of complications.
