About
Decreased neutrophils causing an impaired immunity response, thus may become very ill and debilitated from common viruses and bacteria.
Assess
Decreased neutrophils, possible decreased wbc's, history of chemotherapy treatment, fatigue, fever, exposure to infection.

Diagnosis
Risk for infection related to impaired immune response
Goals
- The client will be free from infection.
- The client will state three ways to conserve energy.
Interventions
- Utilize strict handwashing before entering clients room and before any tests, procedures requiring client contact.
- Monitor for signs and symptoms of infection.
- Post signs indicating precautions for staff to utilize before entering room.
- Encourage client to mask when in crowds; teach to avoid crowds or people with infections.
- Teach client to monitor for signs and symptoms of infection.
- Teach client proper neutropenic precautions including avoiding fresh fruits, vegetables, plants; avoiding sick individuals; strict handwashing to prevent infection.
- Monitor for signs/ symptoms of complications.
