199-leukopenia

About
Decrease in leukocytes either from lymphopenia ( decreased lymphocytes) or neutropenia ( decreased neutrophils).
Assess
CBC with differential will show abnormally low neutrophils or lymphocytes. Patient may present with infection or opportunistic infection. Neutropenia is common in the weeks following administration of chemotherapy.
Complications
Infection, opportunistic infection, sepsis, death
Diagnosis
Risk for infection related to altered body defenses.
Goals
  • The client will remain free form infection.
  • The client will identify signs and symptoms of infection.
  • The client will identify methods to decrease the risk of infection.
Interventions
  • Assess vitals, CBC, lung sounds.
  • Monitor for signs and symptoms of infection.
  • Perform hand hygiene when entering and leaving patient room.
  • Teach client methods to prevent infection including hand-washing, avoiding large crowds, monitoring temperature.
  • Teach client to avoid fresh flowers, plants, litter boxes or other potential sources of contaminants with low resistance.
  • Maintain patient on reverse isolation to prevent opportunistic infections.
  • Encourage coughing and deep breathing exercises and incentive spirometry to prevent respiratory infections.
  • Teach client signs and symptoms of infection or complications to report immediately to health care provider.
