About
An either temporary or permanent secretion of excessive thyroid hormones; leads to an increased metabolism by tissues and organs all over the body.
Assess
Enlarged thyroid gland ( goiter); bulging eyes, sweating, diaphoresis, heat intolerance, increased metabolism, weight loss, photophobia, vision changes; nervousness, jittery, hyperactive, manic.
Complications
Thyroid storm; seizures, coma, death
Diagnosis
- Impaired skin integrity related to edema and dry, flaky skin
- Imbalanced nutrition: less than body requirements related to increased metabolic demands
- Anxiety related to hormone changes leading to excitability, restlessness
Goals
- The client will state reasons to maintain current medical treatment.
- The client will identify ways to manage heat intolerance.
- The client will maintain skin integrity.
- The client will identify ways to maintain adequate nutrition.
Interventions
- Assess VS, cardiac rhythm.
- Monitor for s/s thyroid storm ( temp. increase, tachycardia, systolic hypertension, psychosis, confusion, seizures) and report promptly to physician.
- Administer medications ( iodine, thyroid lowering drugs, beta blockers, etc.) as ordered.
- Keep the room quiet and reduce environmental stimuli.
- Encourage cool, tepid baths or sponge baths frequently to keep cool.
- Monitor for signs/ symptoms of complications.
