About
High blood pressure of systolic greater than or equal 135 mm hg and/ or diastolic greater than or equal to 85 mm hg.
Assess
BP > or = 135 systolic or 85 diastolic; genetic history, dietary intake, alcohol consumption, smoker, hx. Of cardiovascular disease, headaches, dizziness, syncope events,
Complications
Stroke, arteriosclerosis, atherosclerosis, myocardial infarction, heart failure
Diagnosis
- Ineffective health maintenance related to deficient disease and medicine knowledge
- Risk for ineffective therapeutic regimen management related to possible noncompliance with treatment
Goals
- The clients blood pressure will be within normal ranges.
- The client will be able to state treatment modalities.
Interventions
- Monitor vital signs, triglycerides, cholesterol levels
- Administer antihypertensives as ordered.
- Teach client non-pharmacological treatments including low sodium, low fat, low cholesterol diet, exercise, weight reduction, smoking cessation, avoiding alcohol, decreased stress.
- Teach client about medications including rationale, side effects, dosing schedule.
- Teach client how to monitor blood pressure and when to seek follow-up treatment.
- Teach client about hypertension and its effects on other body areas.
- Monitor for signs/ symptoms of complications.
