128-Peptic Ulcer Disease

About
Lesion in the mucosa of the stomach or duodenum.
Assess
Abdominal pain, discomfort, dyspepsia ( heartburn), hx of abdominal disease or h. pylori infection. Assess for frequent use of NSAIDS, alcohol, or tobacco.
Complications
Gastrointestinal bleeding- Monitor stools, test for occult blood, monitor for coffee-ground emesis, assess vs. May need an NG tube and/or transfusion if actively bleeding, thus notify physician.
Diagnosis
  • Pain r/t ulceration of the gastrointestinal mucosa
  • Knowledge deficit r/t lack of contributing factors for peptic ulcer disease
Goals
  • Client will identify three ways to decrease pain and exacerbation of peptic ulcer disease.
  • Client will demonstrate understanding of medication regime to help with PUD.
Interventions
  • Monitor stools for occult blood, assess vital signs and bowel sounds.
  • Administer analgesics as ordered.
  • Administer medications ( PPI's, Antacids, Mucosal protectants, Prostaglandin Analogues, Histamine2 Inhibitors, etc.) as ordered by and explain to client rationale for use.
  • Teach client risk factors for the development of PUD.
  • Teach client low-residue bland diet.
  • Monitor for signs/ symptoms of complications.
