122-Irritable Bowel Syndrome (IBS)

About
Combination of chronic and recurrent constipation, nausea, diarrhea, and/ or bloating.
Assess
Thorough bowel assessment, including history, bowel elimination habits, dietary history, food intolerances.
Complications
Depression, ineffective coping- refer to support services in area, encourage client to notify health care physician if changes or difficulty managing symptoms.
Diagnosis
Pain r/t spasms in gastrointestinal tract
Constipation r/t stress, causative factors
Goals
  • Client will have normal bowel elimination.
  • Client will state a decrease in pain using 1-10 pain scale.
Interventions
  • Monitor stool consistency, quantity, color.
  • Encourage client to increase dietary fiber, allow client to choose foods that are high in fiber.
  • Encourage relaxation techniques and stress reduction techniques.
  • Administer medications as ordered ( anticholinergics, laxatives, 5-HT4 ( Zelnorm), antispasmodics, etc.).
  • Teach client to keep a food diary to help identify causative agents that precipitate events.
  • Monitor for signs/ symptoms of complications.
