About
Protrusion of parts of the stomach through the diaphragm and into the thorax. Asymptomatic or symptoms compare to GERD.
Assess
Regurgitation, heartburn, pain, difficulty with swallowing. Chest pressure may also accompany, diagnostics would need to be available to ascertain a gastrointestinal vs. cardiac origin for chest discomfort.
Complications
Surgical intervention- prepare client for surgery, teach post-operative breathing exercises, monitor for post-operative complications, maintain NG tube post-operatively as ordered by physician.
Diagnosis
Pain r/t anatomical abnormality
Goals
- Client will identify three strategies to manage symptoms from hiatal hernia.
- Client will state a decrease in pain on a 1-10 scale.
Interventions
- Encourage client to remain sitting up after meals.
- Teach the client preventative strategies to help with hiatal hernias including remaining upright, avoiding late-night meals, avoiding tight clothing, avoiding acidic foods, eating smaller meals.
- Teach client about medications prescribed by physician as ordered ( antacids, histamine antagonists, etc.).
- Monitor for signs/ symptoms of complications.
