About

Assess
Abdomen, bowel sounds, distention, nausea, vomiting, last bowel movement, pain.
Complications
- Hypokalemia-administer replacement potassium with an infusion pump slowly via IV.
- Fluid volume deficit- maintain fluid replacement with IV fluids.
- Peritonitis- monitor for rigid abdomen, increased pain, fever, tachycardia, pain.
Diagnosis
- Pain r/t abdominal pressure and distention
- Constipation r/t intestinal obstruction
Goals
- Client will state a decrease in pain or pain management effective.
- Client will pass normal soft, formed stool.
Interventions
- Assess bowel sounds, VS, NG Tube placement.
- Maintain NPO until patient has normal bowel sounds or passes flatus and/or stool.
- NG tube to suction for stomach decompression.
- Administer medications to facilitate resumption of peristalsis.
- Prepare for surgery if mechanical obstruction must be removed.
- Administer IV fluids to replace GI losses.
- Administer antiemetic as prescribed by physician.
- Monitor electrolytes for hypokalemia and other abnormalities.
- Monitor for signs/ symptoms of complications.
