About
An alternative bowel elimination route via a stoma on the abdomen secondary to bowel surgery, blockage, tumors, resection.
Assess
- Abdomen, bowel sounds, stoma, stool characteristics ( the higher in the colon, the greater likelihood for loose stools), skin surrounding stoma, ostomy appliance, patient perceptions
Complications
- Necrosis/ gangrene- always keep stoma covered with ostomy appliance or moist sterile saline gauze.
- Skin breakdown- use protective barrier wipes and skin protectants around stoma, clean with water, keep skin clean and dry.
Diagnosis
- Body image disturbance r/t illness treatment
- Skin integrity, impaired ( risk for)
Goals
- Client will maintain skin integrity surrounding stoma on abdomen.
- Client will demonstrate the ability to change ostomy appliance and manage self-care for bowel elimination.
Interventions
- Assess bowel sounds, stool consistency and pattern.
- Monitor abdomen skin for redness, drainage, rash, or swelling.
- Monitor and manage stoma- should be pink, moist. Never leave open to air, cover with moist saline gauze if no ostomy appliance covering, clean with water and soft absorbent cloths. Apply skin barrier before reapplying ostomy appliance.
- Demonstrate to client how to manage ostomy appliance.
- Encourage verbalization of feelings regarding change in lifestyle.
- Educate regarding support groups, ostomy products available.
- Consult with Ostomy/ Wound Care nurse, if one available.
- Monitor for signs/ symptoms of complications.
