About
Inflammation of the appendix, attached to the cecum of the colon in the right lower quadrant of the abdomen.
Assess
Abdomen, fever, wbc?s elevated. May have pain at Mcburney?s point ( midway between umbilicus and anterior iliac crest in the right lower quadrant).
Complications
Perforation of appendix- peritonitis of the abdomen cavity. Avoid laxatives and/or enemas pre-operatively. Avoid applying heat to abdomen area. Prepare for surgery as soon as possible.
Diagnosis
- Knowledge deficit related to surgery
- Risk for infection related to inflammation of the appendix
- Pain related to inflammation and/ or surgery
Goals
- The client will state understanding of pre-operative and post-operative instructions.
Interventions
- Maintain NPO.
- Prepare for surgery.
- Implement incentive spirometry pre-operatively.
- Teach client what to expect post-operatively.
- Apply elastic compression stockings and/or sequential compression device as ordered by physician.
- Encourage to do IS post-operatively at least ten times an hour.
- Teach abdominal splinting.
- Teach client signs and symptoms requiring immediate follow-up by a health care provider.
- Monitor for signs/ symptoms of complications.
