204-Sickle Cell Disease

About
Impaired red blood cells causing changes in hemoglobin, cascading effect produces more impaired hemoglobin and inadequate oxygenation of tissues.
Assess
Recent exposure to sick people, history of disease or genetic link to disease, fatigue, shortness of breath, pain, pallor, cyanosis, sickling of RBC's.
Complications
Sepsis- systemic infection
Multi-organ dysfunction
Diagnosis
  • Fatigue related to disease process
  • Activity intolerance related to decreased oxygen supply to body tissues
  • Anxiety related to knowledge deficit of disease process and treatment
  • Acute pain related to impaired oxygenation to tissues
Goals
  • The client will be free from injury.
  • The client will state three ways to conserve energy during acute exacerbation.
  • The client will be able to identify treatments and rationale for use.
  • The client will state an acceptable pain level after interventions.
Interventions
  • Monitor for signs and symptoms of infection.
  • Monitor vital signs, pulse oximetry, and pain levels every four hours.
  • Assess client frequently.
  • Administer oxygen, as ordered, to maintain adequate oxygenation and to prevent hypoxemia.
  • Administer analgesics, as ordered, as necessary for pain.
  • Encourage adequate hydration and administer IV fluids as ordered.
  • Encourage client bedrest during acute exacerbation
  • Teach client to monitor for signs and symptoms of infection.
  • Monitor for signs/ symptoms of complications.
