177-Cushings

About
An increase in cortisol levels related to the increased stimulation of the adrenalcorticotropic hormones ( ACTH).
Assess
Assess for bone pain and injuries, assess for bruising, history of infections and history of stomach ulcers, thin fragile skin, hyperglycemia, high blood pressure, weight gain. Common assessment findings include osteoporosis, humped back, moon face, large trunk area, thin skin, and edema.
Complications
GI bleeds; infection; ulcers; pulmonary edema; dysrhythmias
Diagnosis
  • Excess fluid volume related to failure of regulatory mechanisms
  • Activity intolerance related to weakness and fatigue
Goals
  • The client will have acceptable fluid balance.
  • The client will remain free from injury.
  • The client will be able to tolerate activities of daily living.
Interventions
  • Monitor daily weights, VS, ECG's, intake/ output, and labs.
  • Monitor for s/s of fluid overload, especially pulmonary manifestations.
  • Administer medications as ordered.
  • Institute methods to reduce the risk of injury.
  • Encourage client to change positions frequently ( at least every two hours) to minimize risk of pressure ulcers.
  • Teach client methods to reduce the risk of infection.
  • Prepare for surgery, if indicated.
  • Monitor for signs/ symptoms of complications.
