138-Atrial Fibrillation

About
Arrhythmia characterized by chaotic atrial activity with irregular ventricular response with resulting irregular pulse and rhythm. May cause sluggish blood flow through the heart leading to possible blood clot formation.
Assess
Fatigue, weakness, shortness of breath, activity intolerance, chest pressure or pain, hypotension, pulse oximetry
Complications
  • Pulmonary embolism, MI, stroke, decreased cardiac output.
Diagnosis
  • Decreased cardiac output related to abnormal heart rate or rhythm
  • Ineffective tissue perfusion related to altered blood flow through the heart
Goals
The client will remain free from complications related to arrhythmia.
Interventions
  • Monitor vital signs, triglycerides, cholesterol levels, electrolytes, coagulation studies, cardiac enzymes, telemetry.
  • Monitor for dizziness, syncope, chest pain.
  • Monitor neurological status frequently.
  • Administer medications ( anticoagulants, antiplatelets, antidysrhythmics, etc.) as indicated.
  • Administer oxygen and monitor pulse oximetry.
  • Prepare for procedures ( cardioversion, ablation) as indicated.
  • Teach client about treatments, medications, and s/s to report to health care provider immediately.
  • Monitor for signs/ symptoms of complications.
