About
Percutaneous transluminal coronary angioplasty: a technique that allows for re-perfusion of cardiac tissue by inserting a balloon to open up an occluded area of a coronary artery.
Assess
Insertion site, bleeding, hypotension, hypokalemia, arrhythmias, vital signs frequently
Complications
Re-stenosis, bleeding, clots, dysrhythmias, heart attack
Diagnosis
- Decreased cardiac output related to altered blood flow through the coronary arteries
- Fear related to invasive cardiac procedure
Goals
- The client will have return of blood flow through artery.
- The client will remain free from complications related to procedure.
- The client will state understanding of pre-, intra-, and post-operative activities.
Interventions
- Monitor vital signs, telemetry, intake and output pre-, intra-, and post-operatively.
- Teach client what to expect pre-, intra-, and post-operatively.
- Post-operatively, monitor VS every 15 minutes X 2, every 30 Minutes X 2, every 1 hour X 2, than every 4 hours ( may have different guidelines for each clinical agency).
- Monitor site for signs of bleeding and monitor for angina, electrolyte imbalances, arrhythmias, and hypotension.
- Administer medications ( nitrates, anticoagulants, antiplatelets, antidysrhythmics, etc.) as ordered.
- Teach client side effects and adverse reactions of medication therapy.
- Teach client lifestyle changes and discharge care.
- Prior to procedure, administer medications ( plavix, etc.) as indicated.
- Monitor for signs/ symptoms of complications.
