About
Increased frequency of bowel movements usually several times a day with a liquid consistency.
Assess
Urgency, incontinence, pain, cramping, abdominal discomfort, onset, precipitating and alleviating factors.
Complications
Cardiac arrhythmias, Severe electrolyte depletion, hypotension, orthostatic changes.
Diagnosis
- Fluid volume deficit related to intestinal losses.
- Impaired skin integrity related to digestive enzyme irritants.
- Risk for injury related to complications of dehydration.
Goals
- The client will maintain adequate fluid balance.
- The client will report a decrease in abdominal discomfort and bowel movements.
- The client will identify ways to alleviate skin irritation.
Interventions
- Assess vital signs, electrolytes, intake and output frequently.
- Monitor stool consistency, color, amount, odor and send for laboratory analysis as indicated.
- Monitor cardiac rate and rhythm if electrolyte imbalance.
- Replace fluids and electrolytes as ordered by a physician.
- Teach client about foods that could increase or decrease the severity of symptoms.
- Apply barrier cream to excoriated skin.
- Administer antidiarrheals, antibiotics, antiinflammatories as indicated by a physician.
