Diagnosis

- Acute pain related to irritation of urinary tract
- Impaired urinary elimination related to irritation of urinary tract
Goals
- The client will be free from infection.
- The client will be able to identify contributing factors to condition, and methods to prevent reoccurrence.
Interventions
- Monitor urinalysis, vitals, and s/s of infection.
- Monitor ability to void, retention of urine, other factors that may increase risk for infection.
- Monitor resolution of symptoms, BUN, creatinine.
- Encourage client to utilize effective hygiene measures to prevent bacterial invasion ( wipe front to back, etc.).
- Teach client to not use soaps or hygiene sprays in perineal area that may contribute to tissue irritation.
- Administer medications ( antibiotics, antispasmodics, etc.) as ordered.
- Encourage client to drink extra fluids and well balanced diet to help prevent infection.
- Use surgical asepsis when initiating an indwelling catheter.
- Monitor for signs/ symptoms of complications.
