8-Nephrosclerosis

About
Thickening of the nephrons resulting in kidney damage and hypoxia. Linked to Hypertension and other chronic conditions such as diabetes.

Assess
High blood pressure consistently > 160/110; Chronic diseases; progressive decline in renal function requiring dialysis if hypertension not controlled.

Complications
End stage renal disease- dialysis or transplant; hypertension; hyperkalemia
End stage renal disease- dialysis or transplant; hypertension; hyperkalemia

Diagnosis
  • Activity intolerance related to fatigue
  • Knowledge deficit related to disease process


Goals
The client will identify strategies to manage hypertension.

Interventions
  • Monitor vital signs, especially blood pressure.
  • Monitor electrolytes, intake and outputs, and daily weights.
  • Administer antihypertensive agents, diuretics, etc. as ordered.
  • Teach client about medications to manage hypertension, stressing the importance of compliance on maintaining kidney function.
  • Teach client other measures to maintain heart health and lower blood pressure ( cardiac diet, sodium restriction, exercise, etc).
  • Monitor for signs/ symptoms of complications.
