About
Thickening of the nephrons resulting in kidney damage and hypoxia. Linked to Hypertension and other chronic conditions such as diabetes.
Assess
High blood pressure consistently > 160/110; Chronic diseases; progressive decline in renal function requiring dialysis if hypertension not controlled.
Complications
End stage renal disease- dialysis or transplant; hypertension; hyperkalemia

Diagnosis
- Activity intolerance related to fatigue
- Knowledge deficit related to disease process
Goals
The client will identify strategies to manage hypertension.
Interventions
- Monitor vital signs, especially blood pressure.
- Monitor electrolytes, intake and outputs, and daily weights.
- Administer antihypertensive agents, diuretics, etc. as ordered.
- Teach client about medications to manage hypertension, stressing the importance of compliance on maintaining kidney function.
- Teach client other measures to maintain heart health and lower blood pressure ( cardiac diet, sodium restriction, exercise, etc).
- Monitor for signs/ symptoms of complications.
