About
Inflammation of the bronchioles resulting in narrowed airway.
Assess
Lung sounds, rhonchi, crackles, previous medical and medication history, history of recent infection, sputum ( culture, color, consistency), respirations, ease of breathing, exertional dyspnea, history of smoking.
Complications
Infection- monitor for yellow-green sputum, increase in temperature, fatigue, and lethargy.
Acute respiratory distress- encourage client to report change in symptoms that require medical intervention.
Diagnosis
- Impaired airway clearance related to excessive, thick secretions
- Acute pain related to chest congestion and bronchospasm
Goals
- The client will have clear lung sounds.
- The client will cough and deep breathe at least ten times an hour.
- The client will be able to state three interventions to help with disease process.
Interventions
- Assess lung sounds at least every shift.
- Monitor pulse oximetry for changes in oxygen status. Monitor VS, especially respiratory rate and depth, for acute changes.
- Monitor for signs and symptoms of worsening or infection.
- Administer oxygen as prescribed by physician to alleviate hypoxia or hypoxemia.
- Administer medications as prescribed.
- Encourage coughing and deep breathing frequently.
- Encourage client to keep sputum for a specimen, if needed, and to note color, consistency, and characteristics of sputum.
- Teach client about medication regimen, coughing and deep breathing, and signs and symptoms to notify health care provider with.
- Monitor for signs/ symptoms of complications.
