About
Large pocket of pus surrounding and encapsulating the tonsils and the pharynx, may be a result of ineffective treatment for tonsillitis.
Assess
Pharynx for large pus pockets, inflammation, and patent airway. Monitor temperature, respirations, signs and symptoms of worsening including impaired airway.
Pain with or without swallowing, difficulty swallowing.
Complications
Airway obstruction, need for immediate intubation, may need surgery.
Diagnosis
- Impaired gas exchange related to partial or complete airway obstruction
- Pain related to inflammatory changes of tissue
- Risk for infection related to infectious process in the oral pharynx
- Risk for ineffective breathing pattern related to inflammation, swelling of airway.
Goals
- Client will maintain a patent airway.
- Client will be able to list medication regime.
- Client will be able to list signs and symptoms of worsening that may require immediate medical attention.
Interventions
- Administer antibiotics as ordered.
- Teach client about antibiotics, stressing the importance of completing entire course even if symptoms improve.
- Teach client signs and symptoms that may require immediate medical attention, including difficulty in breathing, lack of improvement in symptoms after 24-36 hours of antibiotic therapy.
- Prepare the client for incision and drainage of abscess as indicated.
- Prepare client for tonsillectomy as indicated.
- Monitor for signs/ symptoms of complications.
