11-Pyelonephritis

About
Kidney infection.

Assess
Flank pain, fever, chills, fatigue, abdominal discomfort, nocturia, tachycardia, CVA tenderness, burning, urgency, frequency with urination

Complications
Renal failure; nephrectomy; surgical removal of stones; sepsis- promptly notify physician if temperature increases or does not respond to treatment.

Diagnosis
  • Acute pain related to irritation of urinary tract
  • Impaired comfort related to chills and fever
  • Ineffective health maintenance related to deficient knowledge of self-care with disease and treatment


Goals
  • The client will be free from infection.
  • The client will be able to identify treatment strategies with rationale for use.


Interventions
  • Monitor VS ( especially blood pressure and temperature), urine output and characteristics.
  • Monitor for signs and symptoms of infection not responding to treatment.
  • Administer antibiotics as ordered.
  • Increase fluids to 2-3 Liters per day.
  • Collect urine culture and sensitivity and/ or blood cultures as ordered.
  • Teach client medication regimen, disease management, and signs/ symptoms of reoccurrence.
  • Prepare for surgery if client has an obstruction causing kidney infection.
  • Monitor for signs/ symptoms of complications.
