About
An immunologic disorder of the glomeruli resulting in damage to the glomeruli; can be from many different causes, including ineffective treatment of a previous infection, lupus erythema, diabetic nephropathy, and/ or a genetic link.
Assess
Recent infections, pulmonary congestion, fluid overload, dyspnea, crackles, reddish-brown urine, hypertension, fatigue
Complications
Renal failure; rapidly progressive Glomerular nephritis
Diagnosis
- Excess fluid volume related to impaired renal function
- Acute pain related to pressure on internal organs
- Impaired urinary elimination related to irritation of urinary tract
- Ineffective health maintenance related to deficient knowledge of self-care with disease and treatment
Goals
- The client will be able to identify signs and symptoms of complications.
- The client will be free from infection.
Interventions
- Monitor vitals, Intake and output, daily weights, electrolytes, BUN, Creatinine.
- For hypovolemia, monitor lung sounds and administer diuretics, antihypertensives, etc. and fluid restriction as ordered.
- Administer medications ( antibiotics, etc.) as ordered.
- Maintain a quiet environment, encouraging client rest during convalescent period.
- Teach the client about medication therapy for home use, encouraging client to continue antibiotics until gone even if symptoms improve.
- Teach the client how to monitor fluid balance, including initiating daily weights and self-assessment of blood pressure.
- Teach the client s/s to report immediately to health care provider.
- Monitor for signs/ symptoms of complications.
