55-Sarcoidosis

About
Granulomas infiltrating areas of the body, commonly the lungs, causing inflammatory changes that can result in stiffened scarred pulmonary tissue.
Assess
Assess for shortness of breath, cough, blood tinged sputum, chest pain or discomfort, history of abnormal x-rays.
Complications
Pulmonary fibrosis- treat infections promptly.
Multi-organ involvement- monitor serum blood levels for CBC, creatinine, calcium, etc as ordered.
Diagnosis
  • Impaired gas exchange related to changes in lung tissue
  • Anxiety related to change in health status

Goals
  • The client will state signs and symptoms that may represent disease progression or worsening.
  • The client will be able to identify complications of steroid therapy and how to avoid those complications.

Interventions
  • Administer corticosteroids as ordered.
  • Administer other medications, as ordered, to manage symptoms of pain, cough, dyspnea, etc.
  • Encourage client to express feelings regarding disease diagnosis.
  • Teach client techniques to maintain lung compliance, i.e. coughing and deep breathing and early treatment for infections.
  • Educate client about signs and symptoms of disease progression or worsening.
  • Teach the client side effects of steroid therapy and need for gradual discontinuance to avoid complications.
  • Monitor for signs/ symptoms of complications.
