About
Diversionary pathways from the kidneys or bladder to the outside of the body made surgically to bypass damaged or excised lower urinary tract structures. Typically, these diversionary pathways exit from the abdomen into a urinary collection device attached to the abdomen.
Assess
Stoma size, color; appliance; urine characteristics; skin surrounding stoma; client coping
Complications
Impaired skin Integrity; social isolation; disease specific complications
Diagnosis
- Body image disturbance related to presence of stoma
- Knowledge deficit related to care of stoma
- Risk for impaired skin integrity related to ostomy appliances
Goals
The client will be able to demonstrate care of stoma and urinary appliances.
Interventions
- Teach the client what to expect pre-, intra-, and post-operatively for urinary diversions.
- Encourage client to verbalize feelings related to diagnosis and treatments.
- Teach the client how to manage urinary pouches, reservoirs using aseptic techniques.
- Encourage frequent emptying of pouches to prevent overfilling.
- Teach client about medications to manage disease process as ordered.
- Observe client demonstrating care of the urinary diversion.
- Monitor for signs/ symptoms of complications.
